The Behavioral Home Health Program is designed for the patient who cannot always use the traditional outpatient network; the nature of their mental illness or a physical disability prohibits their participation.

Home health care promotes and maintains the patient's independence within his or her own community. It can stop “The Revolving Door Syndrome” often associated with the mentally ill patient.

The Behavioral Home Health Program includes:

  • Case management
  • Medication administration and management
  • Family support
  • Ongoing assessment and monitoring of status
  • Individualized treatment planning
  • Individual one-on-ones with a psychiatric registered nurse
  • A linkage of services coordinated to treat “The Whole Patient”

    The Quality of Life Behavioral Home Health Program is committed to increasing the functional ability and promoting the independence (with or without assistance) of the patient within their own environment.

    A multi-disciplinary team approach is utilized to focus on the immediate and projected needs of the patients under our care.

    Medicare Eligibility

    Criteria for Medicare Eligibility Include:

  • Home health services must be medically necessary and ordered by a physician
  • A need for skilled, intermittent services from licensed professionals
  • A new medical diagnosis or a change in an existing medical condition

    For those individuals who qualify for home health under Medicare guidelines, services are 100% covered

    Candidates for Therapy

    Our psychiatric nursing specialists work with the physician, hospital staff and others to identify the many types of patients who can benefit from this home care program.

    Psychiatric home care is appropriate for patients who are experiencing a psychiatric response to trauma, loss, or terminal illness; who have newly diagnosed mental illness and are candidates for home care; or who have experienced past failures in the psychiatric system and need assistance during crisis periods to avoid or shorten hospitalization. Some of the diagnoses
    might include:

  • Depression
  • Alzheimer's Disease
  • Schizophrenia
  • Bipolar Disorder
  • Organic Affective Disorder
  • Substance Abuse

    Behavioral Health Services

    Psychiatric home care is provided under physician supervision. Our comprehensive program includes any or all of the following services:

  • In-depth mental health and physical assessment
  • Coordination of care and case management by expertly trained staff
  • Compassionate, specialized care, designed to meet the patient’s social and emotional needs
  • Confidential patient monitoring with routine communication to physician, patient and caregivers
  • Formalized, systematic quality improvement
  • Medication-management, including venipuncture
  • Ongoing therapy and treatment education for the patient and family
  • Occupational therapy
  • Crisis intervention and grief and loss counseling
  • Complete patient qualification and reimbursement information services
  • Outpatient detoxification support
  • Coordination of services with community health services

    Our program is directed by a team of nurses qualified to deliver home psychiatric care, coordinating with the psychiatrist who takes an active role in program management.
    Standards of Care
    Our psychiatric home care program follows stringent standards of care which meet federal, state and local law; Joint Commission on Accreditation of Health Care Organization (JCAHO) standards; and other practice standards which relate to the care of patients with mental illness.
    Confidentiality
    Patient records are confidential and maintained according to applicable laws and regulations. Confidentiality is essential with care plans and progress communicated only among the patient, physician and caregivers.
